This Pain Tracker has been designed to help people who are struggling to understand any regular, on-going or chronic pain they may have in their bodies.
It is undated so you can use it whenever you have pain, so no wasted pages and each day is a double page spread featuring:

There's also a double page for appointments relating to your pain so you can keep track of who you see and the outcomes of each appointment. Plus an extra couple of note pages for you to write down any other information you think may be relevant.
Whilst we cannot claim that this tracker is a diagnostic tool, it can help you to help yourself and also keep health professionals more informed so they can create a treatment plan that is best suited to your specific needs.
